The Utah Department of Public Safety (DPS) is responsible for keeping and issuing out of driving records (motor vehicle record) in Utah. The Utah DMV offers two types of driving records the non-certified driving record and the certified driving record. Your Utah driving records contain information about your: Traffic citations, violations, and convictions, alcohol and drug violations, driver license suspensions and revocations.

You can order for a copy of your driving record online, by mail or in person. To order for a copy of your  driving record online, you have to visit the Utah DPS website and provide your: Full name, date of birth, your Utah driver’s license number, issue date and your Social Security number (SSN). To order your own Utah driving record in person or by mail, you will need a completed Request for MVR (Form DLD60) or a completed Request for Records (Form DLD 266M), if you are requesting for a certified driving record. If you are ordering in person, submit the form at your local UT DPS office. However, if you are requesting for a certified driving record, you have to submit it at the Administrative office:

4501 S 2700 W, 3rd Floor, Salt Lake City.

If you are ordering by mail, mail the form to:

You can order for another driver’s driving record if your request falls under the following authorized uses:  Government agencies, courts conducting official business or  law enforcement agencies, Attorneys,  Insurance agencies investigating claims or adjusting policy premiums and people with written consent from the driver on the MVR.
