You can order for your driving record, also called an official driving record (ODR) from the Department of Public Safety and Corrections (DPS) Office of Motor Vehicles (OMV). Your driving record contains your driving history and can be used to adjust your car insurance policy rates, in legal proceedings and to verify personal information for pre-employment screenings and background checks. To order for your driving record in person or by mail, you have to complete the Louisiana ODR Request Form by filling Form R0800. Then mail your application and payment to the Office of Motor Vehicles, or submit your request at any of the following Louisiana DMV offices: Alexandria, Baton Rouge, Lake Charles, Monroe or Shreveport.
